Honorarium Payment Request
The Honorarium Payment Request Form has replaced the submission of the Check Request Form and the types of contracts below when the services provided are by non-Suffolk employees, on a one-time basis, with no responsibility for teaching, and payment is not more than $1,000.*
- Visiting Speaker/Lecturer/Panelist Agreement
- Art & Design Visiting Speaker/Artist/Critic Agreement
- Art & Design Thesis Advisor Agreement
- Competition Judge Agreement
*If honorarium payment is going to be more than $1,000 a Speaker/Lecturer/Panelist or Independent Contractor Agreement will be needed. In this case, please contact the Office of General Counsel to discuss.
If a provider is a non-U.S. citizen the Business Office needs to be contacted two weeks in advance. Consult the New Vendor Tax Form Requirements page for further details. Also, if a provider is presenting the same material to more than one section of the same course, both dates can be entered on one form.
If payment needs to exceed the $1,000 limit, an Independent Contractor Agreement may be required. In this case, please contact the Office of General Counsel to discuss.
