Each month, we ask you to report your vacation and health absence time used in the month prior.
The form is due to the Payroll Office by the 10th day of each month. Download it and follow these instructions for filling it out:
- Complete the report on the last day of each month
- Obtain your supervisor’s signature
- Forward the completed report to the Payroll Office (by fax, interoffice, or in person)
All time taken will be deducted from your balance. Remember to keep your record up-to-date and file it on time. If you are at the maximum vacation accrual and your report is not filed on time, you may lose some vacation accrual.

Notes for new employees
If you began employment before the 15th day of the month, you will accrue vacation time for that month. If you began after the 15th day, your accrual begins on the first day of the following month. You are not eligible to use accrued vacation time until after 3 months of employment.
Personal time will be available for use after 3 months of employment.
